. � , . . ��Dw ��o csTY . . �' ISK Grantee Letter of Agreement LeYs Play Playground Joint Use Grant KaBOOM!//Dr Pep�ser Snapple Group City of Redwood City("Grantee") is hereby awarded a grant of$15,000 by KaBOOM!,to appiy toward the creation of a joint use agreement under the terms and conditions stated in this Letter of Agreement (LOA). The grant provides funds for the joint use of playground facilities. Failing to comply with any of the requirements stated in this LOA or providing false information may result in forfeiture of the grant. Scope of Project 1. Grantee will establish a joint use agreement within one year of award date, on or before August 28, 2013. 2. Joint use agreement must include having school playgrounds open for public use outside of regular school hours(to the local neighborhood, at a minimum). 3. The grant must be used to open a minimum of four playgrounds. There is no upper IimiY parameter to this grant. 4. Grant must be formalized in an agreement that may be a joint use policy or a joint use agreement established between the local schooi system and the municipality and/or Parks&Recreation Department. Agreement or policy will establish hours and days of public use and duration of agreement. 5. Joint use agreement must be established for no less than five years. Disbursement and Verification of Grant Funds 6. Grant funds will be disbursed directly to the Grantee in two phases: a) The first half of the grant will be distributed within 30 days of the completion of the Letter of Agreement. b) The Joint Use Grant Final Report will be submitted within 30 days of the implementation of the joint use agreement and no later than 13 months of the Joint Use Grant award date. The Final Report will include: i. written responses to year-end questions; ii. budget detailing grant expenditures[See Definition section for a listing of Allowable Expenses.]; iii. proof of matching funds; iv. copy of policy or memorandum signed by the appropriate representative from each the city and the school; and v. documentation of hours, days, and duration of joint use agreement. c) Awardees will receive the second half of the grant within 10 days of receipt of the Joint Use Grant Final Report. 7. If the terms of the grant are not fulfilled by the appointed deacilines,the Grantee is unresponsive to the KaBOOM! Grants Manager, andlor does not meeting benchmarks of the grant program, it may result in the forfeiture of the grant. Ongoing Participation during Grant Period 8. Grantee agrees to meet the timelines established for the submission and/or completion of the following key project benchmarks.
