_� ����� <br /> EXHIBIT A <br /> FEE SCHEDULE <br /> The Employer shall pay to Flex-Plan the following fees and costs: <br /> 1. Monthly Processing Fees: <br /> For processing and administration,the Employer shall pay to Flex-Plan the following fees: <br /> 1.1 $4.00 per month per Participant that orders through the Program <br /> 2. Summary Plan Description Fee:$3.50 per Summary Plan Description printed and sent. <br /> Provided only upon Employer request. <br /> 3. Meetings and Benefit Fairs: <br /> Attendance at local meetings and benefit fairs beyond the first three(3)are charged at the following <br /> rate with prior notice to the Employer: <br /> 3.1 Employer shall pay to Fiex-Plan$75.00 per hour,or$300.00 per eight-hour day,whichever is <br /> less; <br /> 3.2 Air travel and lodging expenses shall be charged to the Employer at Flex-Plan's cost; <br /> 3.3 Automobile mileage is charged at$.36/mile, plus$37.50/hour driving travel time. <br /> 3.4 Air travel time is charged as a full day cost,of$300.00 per day. <br /> 4.
