EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Consultant shall provide all services necessary for the preparation and implementation <br /> of a Residential Relocation Plan in support of the City's acquisition and disposition of <br /> the property at 1606 Middlefield Road as follows: <br /> Task 1. <br /> Consultant shall coordinate the planning and preparation of the Relocation Plan with <br /> City and any affected or interested parties. <br /> Consultant shall prepare a General Information Notice to be delivered as required by <br /> state law. <br /> Consultant shall prepare a project schedule and budget tracking system to support the <br /> development of the Plan and to assist the City in the implementation of the Plan. <br /> Task 2 <br /> Consultant shall prepare a Draft Relocation Plan for review by City. <br /> Consultant shall prepare a revised Draft Relocation Plan, if necessary, based on City <br /> comments for distribution to the project site occupants for the required 30-day public <br /> review comment period. <br /> Consultant shall, as part of the preparation of the Relocation Plan, attempt to interview <br /> all project site occupants and obtain the information required by law for inclusion in the <br /> Plan. <br /> Consultant shall conduct a replacement resource survey, which survey and results shall <br /> be consistent with state guidelines. <br /> Consultant shall prepare a final Relocation Plan, pursuant to state guidelines, <br /> administer the public comment period and incorporate comments as required by law. <br /> Consultant shall submit to City the final Relocation Plan for adoption, with three printed <br /> copies and one electronic file formatted in the most current version of Microsoft Word. <br /> Tas k 3 <br /> Consultant shall provide the services necessary to implement the approved Relocation <br /> Plan and assist up to four residential tenants to relocate to decent safe and sanitary <br /> housing, as required by state law <br /> Tas k 4 <br /> Additional services includes reimbursement for approved expenses, including travel, <br /> �odging, e�raordinary copying Qr other expenses caused by City request. <br /> ATTY/AGR.2014.156/Relocation Plan Proposal Autotemp <br /> REV:08-25-14 VR <br /> Page 9 of 10 <br />
