EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Asbestos Inspection <br /> Consultant shall perform an interior and exterior inspection, including the roof, to identify <br /> visible, accessible suspect asbestos-containing building materials. <br /> Consultant shall collect samples of suspect materials divided into homogenous areas, i.e., <br /> building materials determined by the inspector to be similar based on their color,texture, and <br /> age. Homogeneous Application symbols to be used for identification of samples coliected <br /> shall be: <br /> T= Thermal System Insulation <br /> M = Miscellaneous Materials <br /> S = Surfacing Materials (troweled or spray applied) <br /> Consultant shall assure that all samples are to be collected by a Certified Asbestos Inspector <br /> and placed in 4-mil plastic bags or similar sealable container. Bulk samples are to be sealed <br /> and then transported to the laboratory for testing. Each sample is to be assigned a unique <br /> coded number. Samples shall be collected, analyzed, and stored under strict chain of <br /> custody protocol. <br /> Consultant shall assure that samples collected are based on protocols supporting the EPA <br /> Asbestos Hazard Emergency Response Act(AHERA). <br /> Consultant shall assure that all sample analyses are performed using Polarized Light <br /> Microscopy with Dispersion Staining in accordance with the Environmental Protection <br /> Agency Method 600/R-93/116, and by the same EPA Method Nurnber 400 Point Count, as <br /> appropriate based on the material samples, at a licensed/accredited laboratory. <br /> Lead-Based Paint Screen <br /> Consultant shall conduct a visual assessment of painted surfaces throughout the building to <br /> locate the presence of deteriorated paint, assess the extent and causes of the deterioration, <br /> and to identify other potential lead-based paint hazards. <br /> Consultant shall conduct a lead-based paint screen of major painted components for lead- <br /> containing and lead-based paint using paint chip analysis. Samples shall be collected in <br /> such a manner as to avoid contamination with wood or brick substrate. <br /> Consultant shall assure that all samples collected are places in appropriate containers, <br /> sealed, labeled, and transported under chain of custody protocols to a licensedlaccredited <br /> laboratory. <br /> Schedule <br /> Upon issuance of a Notice to Proceed by City, Consultant shall mobilize within one week to <br /> perForm all sampling and testing.
