Support Human Service programs that meet the basic human services needs of low <br /> income persons with an emphasis on serving priority needs populations: <br /> HS-1: Meet the basic human services needs of low income persons with an <br /> em hasis on servin riorit needs o ulations. <br /> Description of Services to be Provided: <br /> Investigate a minimum of 85 complaints involving 85 unduplicated disabled or elderly <br /> individuals in licensed care facilities in Redwood Cit . <br /> Primary Quantitative Objectives: <br /> Ombudsman Services of San Mateo County, Inc will serve 85 Redwood City persons. <br /> Hold 2 trainings for new ombudsmen, thereby increasing our volunteer base by 20%, <br /> raising the number of field ombudsmen to 60. <br /> Maintain Facility coverage to meet a minimum of 90% of our facility coverage plan for <br /> City of Redwood City. <br /> Increase the facilities' participation in delivering 'resident focused care,' by providing <br /> education at 2 events at which a presenter will provide training on 'Best Practice <br /> Models,' directly addressing topics that we deem to be system problems. <br /> NOTE:Performance wil!be measured by program objectives and quantitatrve goals.
