Pro ram Cost: <br /> HHW, U-Waste, E-Scra and Shar s <br /> SFD Door-to-Door Flat monthly rate of$0.46 per residence for all collection <br /> MFD Door-to-Door Flat monthly rate of$0.21 per residence for all collection <br /> Sharps Only Mail-back $50.00/gallon (plus tax) <br /> (either SFD or MFD) <br /> paid for directly by <br /> Customers ' <br /> SFD—single-family dwelling residence (i.e., 1-4 units) <br /> MFD— multi-family dwelling residence (i.e., 5 or more units) <br /> N/A—not applicable <br /> Cost is for unlimited pick up in the Agency service area. There is no limit on the number of <br /> times a household can participate in the Program. In subsequent years, costs may be adjusted <br /> for CPI and may be adjusted based on a change in the participating Agency's number of <br /> residences. <br /> Resident Participation Instructions <br /> Door-To-Door HHW Collection Service <br /> Resident's must call Contractor's 1-800 telephone number(i.e., 800-449-7587 —800- <br /> HHW-PKUP) or email Contractor at atyourdoorCc�wm.com to schedule door-to-door <br /> collection service. When residents call to order collection services, Contractor shall <br /> explain the participation instructions and schedule a pick up. Contractor will <br /> subsequently mail to resident a kit-bag and detailed participation instructions. Residents <br /> are entitled to an unlimited number of requests/collections each year. <br /> HHW stop includes collection of: <br /> Inside the kit bag - <br /> • poison, solvent, paint, aerosol cans, household cleaners, antifreeze, used oil filters, <br /> thermometers, garden chemicals, hobby supplies, sharps (in a sealed container), used <br /> motor oil and household batteries. <br /> Plus — <br /> • Three TV's or computer monitors <br /> • Five gallons of used motor oil <br /> • Five gallons of antifreeze <br /> • Five fluorescent tubes/lamps (two or more must be taped together) <br /> • Five automotive batteries <br /> • Twenty-five pounds of E-Scrap and U-Waste <br /> Sharps Mail-Back Service <br /> Resident's must call Contractor's 1-800 telephone number (i.e., 800-449-7587 —800-HHW- <br /> PKUP) or email Contractor at hotline _curbsideinc.com to request sharps mail-back service. <br /> When residents call to order the sharps mail-back service, Contractor will explain the <br /> participation instructions, the cost to participate and subsequently mail the sharps container to <br /> resident.
