EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />Consultant will provide services related to the Middlefield Road Underground Utility <br />District — Joint Trench Project as outlined below. <br />Task No. Description of Task <br />Revise plans based on design changes from utility companies: <br />• PG&E electric substructure drawings <br />• Additional changes received from PG&E estimator <br />• COMCAST design changes <br />• Astound design changes <br />• PG&E Form B addition of trenching restoration costs <br />• PG&E comments on conduit sweeps and trench dimensions <br />• AT&T composite drawing review changes <br />• PG&E fiber optic addition <br />• PG&E design changes (at MacArthur, Cassia, and Douglas) <br />• PG&E trench configuration changes <br />2 Revise plans according to utility companies project limit change on Maple <br />Street and on Manzanita Street <br />3 As required by utility companies, modify design and plan set into Phase 1 <br />from Maple Street to Manzanita Street <br />4 Revise PG&E Form B according to design changes, project limit changes, <br />and changes to reflect new project phasing <br />Consultant shall perform the work for a not -to -exceed amount of $32,797.00, and shall <br />invoice monthly based on the percentage of work completed for each task. <br />REV: 06-30-17 PR <br />Page 3 of 3 <br />ATTY/AGR.Amend No. 1 /Bellecci & Associates, Inc. <br />
