7.1.B. - Page 68 <br /> activities and services performed under this Services Agreement upon <br /> request and with reasonable notice from County or contract areas as shown <br /> on Exhibit B,as well as from members of the public. <br /> 26.2 Provide monthly sheltering, field services, and adoption statistical reporting <br /> to County and Cities. Information contained on said monthly reports shall <br /> be agreed upon by Parties annually. <br /> 26.3 Provide annual reports on : <br /> a) Total rabies vaccinations given to dogs and cats by Contractor <br /> b) Total quarantines(home &shelter)monitored by Contractor <br /> c) Total spay/neuter surgeries provided by Contractor <br /> Information contained on said annual reports shall be agreed upon by <br /> Parties annually. <br /> 26.4 Provide annually an organizational chart showing Contractor staff positions <br /> for all areas of this Service Agreement. <br /> 26.5 Work with County representatives to identify means to best gauge County& <br /> Cities fee structure. <br /> 27. Record Requests <br /> 27.1 Comply with any request by a representative of County for records or <br /> documents. <br /> Nothing in this Services Agreement obligates Contractor to release names, <br /> addresses, phone numbers and/or any other personal, private or other <br /> identifying information that it deems confidential for the purposes of <br /> conducting its business or for maintain individuals'privacy protection. <br /> Contractor agrees to hold harmless and indemnify County and its officer, <br /> agents and employees, against any and all claims, suits or actions of any <br /> kind resulting from any decision by Contractor, or its officers, agents or <br /> employees, to withhold any document or information from any member of <br /> the public. <br /> 27.2 Provide Dangerous or Vicious animal reports prepared by Contractor for <br /> purpose of administrative hearings to the County or City and the animal <br /> owners, and other interested parties so requesting, no less than three days <br /> prior to the scheduled hearing; provided however that Contractor reserves <br /> the right to maintain the confidentiality of any private information as <br /> described in this Services Agreement,Section 13 Hold Harmless. <br /> 64IPage February 5 , 2015 <br /> ATTY/AGR/2015.057/SAN MATEO COUNTY-ANIMAL CONTROL SERVICES <br />
