a. Under the supervision of the Sequoia Teen Resource Center Di�ector and in collaboration with <br /> the Sequoia High School administration, volunteers will work with about fifty(50) referred youth <br /> during the 2014-15 school year. <br /> b. Complete a baseline profile for each student entering the program. <br /> c. Develop a written action plan to address the specific issues that interfere with each studenYs <br /> academic success. Every plan will contain measurable goals, such as consistent attendance at <br /> classes,completion of assignments,improvement in grades/behavior,etc. <br /> d. Meet at least weekly with each assigned student to revisit the goals of the action plan and to <br /> ensure focus on goal attainment. <br /> e. Provide school staff members who refer the students with weekly updates (e.g., the student's <br /> participation,efforts, issues,and progress). <br /> f. Help students establish contact with onsite and offsite student support services that promote <br /> school connectedness and success. <br /> g. Follow up with referrals for support services to ensure that assigned students have made <br /> connections and are maintaining those connections satisfactorily. <br /> h. Facilitate delivery of services through scheduling, helping with permission slips, and addressing <br /> other apparent barriers,as deemed appropriate. <br /> i. Maintain complete, confidential, and ongoing communication with appropriate school staff <br /> (teachers,counselors, administrators),as well as assigned students. <br /> j. Maintain complete records of steps taken to assist assigned students. <br /> k. Meet regularly with the Director of Teen Resource Center and other AmeriCorps team members <br /> to continue refining the design and implementation of the program. <br /> I. Complete other related duties as assigned by the school administration or the Director of Teen <br /> Resource Center. <br /> 3. Fundina. Redwood City 2020 will pay to Notre Dame a total fee for the volunteer Services in <br /> the amount of six thousand three hundred and fifty dollars ($6,350). The maximum expenditure by <br /> Redwood City 2020 under this MOU is $6,350. No other compensation for the Services will be <br /> allowed,except for items covered by subsequent amendments to this Agreement. Redwood City 2020 <br /> reserves the right to withhold a ten percent(10%)retention until Redwood City 2020 has accepted the <br /> Services spec�ed above. There shall be no additional charge for expenses unless agreed to by the <br /> Parties. Payment shall occur only after receipt by City of invoices suffiaently detailed to include hourly <br /> rates, hours worked, and tasks performed, unless otherwise agreed to by Redwood City 2020. Notre <br /> Dame will provide a detailed invoice,with evidence of progress in achieving defined deliverables when <br /> requesting payment. <br /> 4. Status of Volunteer. Notre Dame's volunteer will perform the Services as an independent <br /> contractor or employee of Notre Dame and not as an employee of City or Redwood City 2020. <br /> 5. Indemnification. Notre Dame will defend, indemnify and hold harmless City and its officers, <br /> agents, employees and volunteers and Redwood City 2020 and its officers, agents, employees, and <br /> volunteers,ftom and against all claims,damages, losses and expenses including attorney fees arising <br /> out of the performance of the Services, caused in whole or in part by the willful misconduct or any <br /> negligent act or omission of the volunteer or Notre Dame, any subcontractor, anyone directly or <br /> ATTY/AGR/2014.243/NORTE DAME MISSION VOLUNTEERS MOU RQ020 2014-15 <br /> REV:03-26-15 MLG <br /> Page 2 of 4 <br />
