<br />SECOND AMENDMENT <br />TO AGREEMENT <br />BETWEEN THE COUNTY OF SAN MATEO AND <br />CITY OF REDWOOD CITY – FAIR OAKS COMMUNITY CENTER <br /> <br />THIS SECOND AMENDMENT TO THE AGREEMENT, entered into this day of <br /> , 20 , by and between the COUNTY OF SAN MATEO, hereinafter called "County," and <br />CITY OF REDWOOD CITY - FAIR OAKS COMMUNITY CENTER, hereinafter called <br />"Contractor"; and <br />W I T N E S S E T H: <br />WHEREAS, pursuant to Government Code, Section 31000, County may contract with <br />independent contractors for the furnishing of such services to or for County or any Department <br />thereof; and <br />WHEREAS, the parties entered into an Agreement for Core Services on June 4, 2013; <br />and <br />WHEREAS, the parties amended that Agreement on April 11, 2014, to add the Cost of <br />Living Adjustment (COLA); and <br />WHEREAS, the parties now wish to further amend the Agreement to provide Core <br />funding in the amount of $9,821 and Sustainability funding in the amount of $33,437 Fiscal <br />Year 2014-2015. <br />NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO <br />AS FOLLOWS: <br />1. Section 1 – the Exhibits and Attachments to this Agreement are amended to read as follows: <br /> <br />Exhibit A – Program/Project Description Revised 1/2015 <br />Exhibit B1- Method and Rate of Payment <br />Exhibit C – Contractor’s Declaration Form <br />Exhibit D – Program Monitoring <br />Exhibit E – Performance Measures <br />Exhibit F – Clarity <br />Attachment I—§504 Compliance <br />2.
