ORIGINAL <br />AMENDMENT NO. 1 TO EXTEND AND AMEND AGREEMENT FOR <br />CLAIMS ADMINISTRATION SERVICES <br />YORK RISK SERVICES GROUP, INC. <br />This Amendment No. 1 is entered into and effective as of the 1st day of July, <br />2018, extending and amending the agreement dated July 1, 2015 (the "Agreement") by <br />and between the City of Redwood City, a charter city and municipal corporation of the <br />State of California ("City"), and York Risk Services Group, Inc., ("Consultant") <br />(collectively, the "Parties") for workers' compensation claims administration services. <br />,1:1111ilf-3161 <br />A. On June 9, 2015, the Parties executed an Agreement for Claims <br />Administration Services; and <br />B. The Parties have negotiated and agreed to amendments to the Agreement <br />described below. <br />NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br />contained herein, the Parties agree as follows: <br />1. That the Agreement, as may have been amended from time to time, is <br />hereby extended for a period of one year ending on June 30, 2019 and Consultant will <br />be paid a not to exceed amount of Three Hundred Seventy Two Thousand, Three <br />Hundred Forty -Five Dollars ($372,345) for the extension term. With this Amendment, <br />the total Agreement amount shall not exceed One Million Four Hundred Forty -Three <br />Thousand Three Hundred Seventy -Five Dollars ($1,443,375). <br />4. All other provisions of the Agreement, as may have been amended from <br />time to time, will remain in full force and effect. <br />5. All requisite insurance policies to be maintained by the Consultant <br />pursuant to the Agreement, as may have been amended from time to time, will include <br />coverage for this Amendment. <br />6.
