Uncollectable Fees <br /> When the applicable fee is uncollectable, but the service has been provided <br /> in the cases where failure to provide the service would result in conflict to the <br /> Contractor's mission and purpose as a humane society for the prevention of <br /> cruelty to animals, and/or where efforts to collect fees would, in Contractor's <br /> estimation, potentially jeopardize safety of its staff, volunteers or facilities, <br /> Contractor shall make what it believes to be a reasonable attempt to collect <br /> the fee from the responsible party. <br /> Contractor will provide responsible party with a form provided by the County <br /> and agreed to by Contractor. Form will acknowledge debt for services <br /> provided by Contractor by executing said form with responsible parties' <br /> signature. Parties shall meet when necessary to agree upon the form. <br /> If fees remain uncollectable, Contractor will send the following to the County <br /> on a monthly basis: <br /> (a) Excel spreadsheet showing the following: <br /> 1. Responsible party's name, address, and telephone <br /> 2. Service provided and fee charged by Contractor <br /> 3. Reason Contractor was unable to collect fee at the time of service <br /> (b) Itemized invoice <br /> (c) Returned check, if applicable <br /> February 5,2015 -31 - <br />
