EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> The scope of work is for the demolition, loading and removal including labor, <br /> equipment, material, salvage value, transportation, legal disposal and disposal <br /> fees described as follows: <br /> 1. Residence (1,500 +I-SF) <br /> 2. Foundation to 3' below adjacent grade <br /> 3. Site debris (Tool sheds, fencing, garbage encountered during site visit) <br /> 4. Site concrete and asphalt paving <br /> 5. Trees and vegetation within property line; One Oak tree to be protected per City <br /> guidelines <br /> 6. Locate utilities (City to identify utilities intended to be protected) <br /> 7. Utility disconnect, safe off, and removal_ Cut and capped at property line. <br /> 8. Dust control including water; based on using onsite source <br /> 9. Traffic and/or pedestrian control as required (limited to flagmen and signage) <br /> 10.Obtain BAAQMD "J" permit and any other governmental or regulatory permits <br /> required by law. <br /> 11.Notification of utility companies regarding disconnect of service and meter <br /> removal <br /> 12.Equipment access for demolitian to be provided by General Contractor. <br /> 13.A11 demolition to be performed in one mobilization. <br /> 14.Clean debris & concrete recycled to meet recycling requirements <br /> 15.Universal waste removal <br /> 16.Construction fence along property line (275 +/-LF); 6 month rental; provide option <br /> to be responsibility of City after demolition completion <br /> 17.Erosion control measures (275 +/-LF wattle, 2 inlets protected) <br /> 18.Rough grade site to even surfiace <br /> 19.Remove transport and dispose of asbestos containing materials as outlines in the <br /> Phoenix Environmental Consulting Survey dated 10-21-15. <br /> 20.Properly isolate and contain interior where abatement will be performed.
