Page 8 of 9 <br />ATTY/AGR/2016/AMENDMENTS/AMEND NO.3 COUNTY OF SAN MATEO <br />REV: 02-11-16 <br />benchmark, County will deduct the amount of $9,821 from the following years’ <br />contracted amount or seek a refund of any portion of the $9,821 in unused funds. <br /> <br />4. The total obligation for FY 2014-15 shall not exceed $83,331. <br /> <br />FY 2015-16: <br /> <br />1. For FY 2015-16 Contractor will be paid $73,510 in general funds. This amount may be <br />paid quarterly or in one lump sum as shown in the prepayment section above. <br /> <br />2. In FY 2015-16, County allocated an additional 3% COLA of $2,205 to provider <br />calculated on the new base of $73,510. The $2,205 COLA amount may be paid <br />quarterly or in one lump sum as shown in the prepayment section above. <br /> <br />3. Contractor is eligible for $9,065 in general funds for FY 2015-16 based on the <br />Memorandum of Understanding (MOU) with the Department of Housing (DOH) and <br />the offset CDBG funds that will be managed by Samaritan House. To receive these <br />funds Contractor must submit an invoice to County upon execution of this amendment. <br />Should Contractor fail to meet the performance benchmark for these funds as outlined <br />in Exhibit A, County may deduct the amount of $9,065 from the following years’ <br />contracted amount or seek a refund of any portion of the $9,065. <br /> <br />4. The total obligation for FY 2015-16 shall not exceed $84,780. <br /> <br />Sustainability Funds <br /> <br />FY 2014-15 <br /> <br />1. At the discretion of the County Manager, Contractor may be eligible to receive a one- <br />time amount up to $33,437 during the term of the Agreement in sustainability funds <br />should they successfully implement the use of the Core Clarity Data system and <br />demonstrate proficiency through satisfactory Performance Measurement data. Funds <br />shall be issued upon confirming implementation and positive review of performance <br />data as approved by the County Manager. <br /> <br />FY 2015-16 <br /> <br />1. At the discretion of the Director of the Human Services Agency, Contractor may be <br />eligible to receive a one-time amount up to $16,718 during the term of the Agreement <br />in sustainability funds following the evaluation of Performance Measurement data for <br />the 1st quarter of FY 2015-16. Funds shall be issued upon confirming implementation <br />and positive review of performance data as approved by the Director of the Human <br />Services Agency. <br /> <br />Total Agreement Obligation <br /> <br />2.
