-- —.�.. � ----..�..� EXHIBIT "A" SCOPE OF SERVICES Funding for this agreement has been granted from the San Mateo County Health System, Department of Behavioral Health and Recovery Services to support the Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. Peninsula Conflict Resolution Center(°PCRC")will work in collaboration with the AOD Abuse Prevention Partnership to build program capacity in supporting positive family relationships between youth and parents/caregivers, and to increase the number of parents and caregivers with increased positive relationships with youth. Specific tasks to be completed include: • Provide at least 2 prevention-focused, evidence-based Parent Project series for families of 45 high at-risk youth (e.g., youth with poor attendance patterns, discipline problems, and/or low grades) at Woodside, Redwood, and Sequoia High Schools, Kennedy Middle School, and McKinley Institute of Technology by June 30, 2016, measuring outcomes by pre-and post-surveys, yielding the following results: 0 86% of parents/caregivers will report sign�cant increase in education, hopefulness, warmth/affection for their children, communication skills, and strategies to prevent alcohol and drug use. • Work with staff at John W. Gardner Center for Youth and Their Communities to identify indicators of success and develop/implement evaluation plan for tracking success and impact of all the above-mentioned projects. • PCRC staff will attend monthly Prevention Partnership Steering Committee meetings. • Ensure appropriate County reporting and data collection requirements are met including but not limited to entering and maintaining monthly CaIOMS records. • Complete other tasks related the Redwood City 2020 AOD Prevention Partnership as assigned and agreed upon by Peninsula Conflict Resolution Center and Redwood City 2020. Consultant will invoice RWC 2020 monthly, providing documentation of actions taken during the preceding month. An initial payment of $2,900.00 (Two Thousand and Nine Hundred Dollars) will be paid to Consultant upon execution of this Agreement. The remaining 80% of compensation will be paid in four equal monthly installments, pursuant to invoice terms above. Unless earlier terminated, the term of this Agreement will be effective from July 1, 2015 until June 30, 2016.
