EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Funding for this agreement has been granted from the San Mateo County Health <br /> System, Department of Behavioral Health and Recovery Services to support the <br /> Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br /> Consultant will work in collaboration with the AOD Abuse Prevention Partnership to <br /> build program capacity in youth serving and mentoring, and to increase the number of <br /> youth involved in positive activities and supportive youth/adult relationships. <br /> Specific tasks to be completed include: <br /> • Consultant staff will collaborate with the Sequoia Youth Advisory Board (YAB) to <br /> update the assessment tool with AOD survey questions; <br /> • Consultant will coordinate with school staff to allow YAB members to go into <br /> classrooms and administer the Needs Assessment Survey with students. Staff <br /> and youth will present the outcomes of this survey to the Prevention Partnership <br /> and administrative staff at Sequoia High School; <br /> • Consultant staff will be trained to implement the Leadership and Resiliency <br /> program and will recruit at least 15 high-risk youth at Sequoia High School to <br /> participate in this program; <br /> • Consultant will facilitate a Leadership and Resiliency program in the 2015/2016 <br /> school year at Sequoia High School, implementing the following 4 program <br /> components: <br /> o weekly resiliency group meetings, including expressive art approaches, <br /> pscyho-education on drugs and alcohol, as well as individual skill-building; <br /> o community service activities; <br /> o peer education; <br /> 0 outdoor activities; <br /> • Work with staff at John W. Gardner Center for Youth and Their Communities to <br /> refine indicators of success and implement evaluation plan for tracking success <br /> and impact of all the above-mentioned projects; <br /> • Consultant's staff will attend monthly Prevention Partnership Steering Committee <br /> meetings; <br /> • Ensure appropriate County reporting and data collection requirements are met <br /> including but not limited to entering and maintaining monthly CaIOMS records; <br /> REV:03-08-16 RL <br /> Page 10 of 11 <br /> ATTY/AGR.2016.042/AOD Subcontract StarVista 2015/16 <br />
