EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Funding for this agreement has been granted from the San Mateo County Health <br /> System, Department of Behavioral Health and Recovery Services to support the <br /> Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br /> Consultant will work in collaboration with the AOD Abuse Prevention Partnership to <br /> ' build program capacity in increasing assets and resilience of vulnerable populations <br /> through youth development, to increase the number of youth and adults in supportive <br /> and caring relationships, and to provide youth development training and support for <br /> adult mentors in the community. <br /> Specific tasks will include: <br /> • Consultant will provide at least 5 prevention-focused, evidence-based trainings to <br /> at least 20 new mentors to work with at-risk youth from Redwood City and North <br /> Fair Oaks. Trainings will highlight the importance of positive familial & other <br /> relationships and encourages pro-social behavior development in mentees. <br /> • Consultant Program Coordinators will provide a minimum of 30 contacts with <br /> caregivers of at-risk youth (from Redwood City and North Fair Oaks) involved in <br /> the program, providing caregivers with increased knowledge of evidence-based <br /> mentoring practices and prevention-focused activities with the intent to support <br /> caregivers mentoring their youth. <br /> • Consultant Program Coordinators will provide regular contact (min. 100 contacts) <br /> with Mentors of at-risk youth from Redwood City and North Fair Oaks to provide <br /> prevention-focused, evidence-based support. <br /> • Continue to improve intemal practices in support of inentors and mentees as <br /> they relate to AOD prevention efforts. <br /> • Work with staff at John W. Gardner Center for Youth and Their Communities to <br /> identify indicators of success and develop/implement evaluation plan for tracking <br /> success and impact of all the above-mentioned projects. <br /> • Consultant staff will attend monthly Prevention Partnership Steering Committee <br /> meetings. <br /> • Ensure appropriate County reporting and data collection requirements are met <br /> including but not limited to entering and maintaining monthly CaIOMS records. <br /> • Complete other tasks related the Redwood City 2020 AOD Prevention <br /> Partnership as assigned and agreed upon by Consultant and Redwood City <br /> 2020. <br /> REV:03-08-16 RL <br /> Page 10 of 11 <br /> ATTY/AGR.2016.043/AOD Subcontract Friends for Youth 2015/16 <br />
