MOW delivers hot, nutritious meals to <br /> program participants 252 days per year, with additional meals provided for <br /> holidays and weekend meals to eligible recipients. <br /> 2) Safety Check. The daily safety and wellness check provided by the delivery <br /> personnel, is vital to the safety and well-being of the participants. It has <br /> proven to be a literal life-saver on many occasions every year! <br /> 3) Assessment and Referral. Each participant is visited in their homes shortly <br /> after they begin receiving meals and then re-assessed every six months. The <br /> MOW Manager discusses needs and living situation with participants and <br /> provides referrals and assistance. Funding Requested: $50,000. Funding <br /> Recommended: $10,000. <br /> Peninsula Volunteers, Inc (PVI) — Rosener House Adult Day Services. <br /> Rosener House Adult Day Services provides a comprehensive program of <br /> services for disabled older adults in a community-based setting. Older adults with <br /> physical and/or cognitive disabilities participate in therapeutic activities, health <br /> and support services, opportunities for socialization, and access to other <br /> community resources. Rosener House provides caregivers respite from their <br /> constant responsibilities. The goal is for each participant to be supported to <br /> maintain their highest degree of independence and well-being, making it possible <br /> for family caregivers to keep them at home, preventing premature or <br /> inappropriate institutional placement. Funding Requested: $20,000. Funding <br /> Recommended: $0. <br /> Project WeHope — Dignity on Wheels Mobile Hygiene Program. <br /> Dignity on Wheels provides shower and laundry services through a mobile trailer <br /> for the homeless living in rotating housing programs, encampments, cars, or on <br /> the street with limited or no access to showers or laundry facilities. By providing <br /> these basic services of clean water and sanitation for free, unsheltered clients <br /> may be able to overcome the barriers that are preventing them from attending <br /> their medical, housing, job or case management appointments. Project <br /> WeHOPE's goal over time is to bring additional services to clients using the <br /> Mobile Hygiene unit through strategic partnerships. Funding Requested: <br /> $85,673.00. Funding Recommended: $27,000.00. <br /> Samaritan House — Free Clinic of Redwood City. The Samaritan House Free <br /> Clinic of Redwood City provides — at no charge— primary, preventive, and <br /> specialty healthcare services for low-income uninsured residents of the Sequoia <br /> Healthcare District. Activities include primary care medical services, dentistry, <br /> endocrinology, diabetic care, gynecology, nephrology, neurology, orthopedics, <br /> ophthalmology, optometry, podiatry, and surgery. Funding Requested: $15,000. <br /> Funding Recommended: $15,000. <br /> Service League of San Mateo County/ Emergency Backpack Project. <br /> Service League will provide emergency relief back packs filled with portable <br /> nourishing food, beverages, and basic hygienic supplies and seasonal <br /> 3 <br />
