EXHIBIT A <br /> CITY OF REDWOOD CITY <br /> Peninsula Volunteers, Inc. <br /> FISCAL YEAR 2015-2016 <br /> Agency Information <br /> Agency Name: Peninsula Volunteers, Inc. <br /> Address: 800 Middle Avenue <br /> City, State, Zip: Menlo Park, CA 94025-5121 <br /> Activity/Program Title: Meals on Wheels (MOW) <br /> Quantitative Goals <br /> Description of Services to be Provided: <br /> PVI Meals on Wheels is the sole provider of home delivered meals to eligible <br /> older adults and adults with disabilities in Redwood City. There are three <br /> components to the program: <br /> 1) Food Security and Healthy Food. MOW delivers hot, nutritious meals to <br /> program participants 252 days per year, with additional meals provided for <br /> holidays and weekend meals to eligible recipients. <br /> 2} Safety Check. The daily safety and wellness check provided by the delivery <br /> personnel, is vital to the safety and well-being of the participants. It has <br /> proven to be a literal life-saver on many occasions every year! <br /> 3) Assessment and Referral. Each participant is visited in their homes shortly <br /> after they begin receiving meals and then re-assessed every six months.
