5.1�_ <br /> I <br /> REPOR T <br /> To the Honorabie Mayor and City Council <br /> From the City Manager <br /> August 9 2004 <br /> Subject <br /> Execution of Funding Agreements for Human Services Financial Assistance Program <br /> (HSFA) 2004/2005. <br /> Recommendation <br /> Authorize the City Manager to execute the Human Services Financial Assistance Program <br /> agreements in the amounts specified in the 2004/2005 fiscal year approved budget. <br /> Background <br /> After reviewing each proposal and hearing presentations from each of the applicants, on <br /> March 17, the Housing and Human Concerns Committee recommended funding as <br /> indicated below. On June 21, the Council instructed the City Manager to include these <br /> recommendations in the FY 04/05 budget ordinance. Council approved the FY 04/05 <br /> budget ordinance on July 26. <br /> Effective July 1, 2004, one of the FY 04-05 grantees, Clara-Mateo Alliance, merged with <br /> another agency, InnVision, The Way Home. All assets of Clara-Mateo Alliance have been <br /> or will be transferred to InnVision, which will also become responsible for any existing <br /> contractual obligations of Clara-Mateo Alliance. InnVision's Clara-Mateo Division will <br /> continue to operate the Elsa Segovia Center. As such, staff is recommending that the <br /> grant and subsequent funding agreement originally approved for Clara-Mateo Alliance's <br /> Elsa Segovia Center be put in the new organization's name, InnVision, The Way Home. <br /> Agreements have been developed with each of the following agencies for FY 04-05 HSFA : <br /> Casa de Redwood/Better Living for Seniors $5,000.00 <br /> Center for Independence of the Disabled $5,000.00 <br /> Community Education Center $5,000.00 <br /> CORA/24 Hour Crisis Line and Support Services $10,000.00 <br /> EI Centro de Libertad/SMART Program $5,000.00 <br /> Ellipse Peninsula AIDS Services $7,900.00 <br /> Family Connections $6,000.00 <br /> Family Service Agency/Senior Case Management and Advocacy $7,800.00 <br /> Friends for Youth/Mentoring Services for Redwood City Youth $5,000.00 <br /> InnVision, The Way Home/Clara Mateo Division: Elsa Segovia Center $5,000.00 <br /> International Institute $15,000.00 <br /> Legal Aid Society/Legal Assistance for At-Risk Redwood City residents $10,000.00 <br /> Milts-Peninsula Senior Focus/RSVP $2,000.00 <br /> Ombudsman Services of San Mateo County $10,000.00 <br /> Peninsula Volunteers/Meals on Wheels $10,000.00 <br /> Peninsula Volunteers/Rosener House Senior Day Care Center $10,000.00 <br /> _ .. � . . _ , <br />
