5. � �-Z <br /> AGREEMENT BETWEEN THE COUNTY OF SAN MATEO <br /> AND THE <br /> CITY OF REDWOOD CITY <br /> THIS AGREEMENT, entered into this day of , 2004, by <br /> and befinreen the COUNTY OF SAN MATEO, hereinafter calied "County," and the City of <br /> Redwood City for the Fair Oaks Community Center, hereinafter called "Contractor'; <br /> WITNESSETH: <br /> WHEREAS, pursuant to Government Code, Section 31000, County may contract <br /> with independent contractors for the furnishing of such services to or for County or any <br /> Department thereof; and <br /> WHEREAS, on May 4, 2004, the Board of Supervisors approved the Community <br /> Development Block Grant (CDBG), Emergency Shelter Grant (ESG), and HOME Action <br /> Plan for FY 2004-05 which included funding for this activity; and <br /> WHEREAS, it is necessary and desirable that Contractor be retained for the purpose <br /> of providing San Mateo County residents with information and referral, emergency <br /> assistance, case management and other services at the Fair Oaks Community Center <br /> (these being referred to collectively in this Agreement as "Core Services"); and <br /> WHEREAS, both parties now wish to enter into an Agreement, to provide the Core <br /> Services for the period of July 1, 2004 to June 30, 2005, for a total obligation of $58,726.00. <br /> NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS <br /> FOLLOWS: <br /> CONTRACT CONTRACT TERM <br /> AMOUNT $58,726.00 Start Date 7/1/04 <br /> End Date 6/30/05 <br /> COUNTY REPRESENTATIVE CONTRACTOR REPRESENTATIVE <br /> Steve Cervantes, Director, Office of Housing Teri Chin <br /> 262 Harbor Blvd., Bldg A 2600 Middlefield Road <br /> Belmont, CA 94002 Redwood City,CA 94063 <br />
