12. Occupancy Reports - Annually, on June 30th of every year of the HOME <br /> affordability period stated in Paragraph 5 of this Exhibit A to the Agreement, Contractor <br /> shall provide to the City a report containing , at a minimum, the following items and <br /> information: <br /> i) Total number of HOME-assisted units that are occupied ; <br /> ii) Total number of HOME-assisted units that are vacant; <br /> iii) A rent roll for all units containing the unit numbers, the lease <br /> commencement and expiration dates, the numbers of bedrooms and <br /> bathrooms within the rented units, the rental rate for each rented unit, <br /> and indicating which units are rented as HOME-assisted units; <br /> iv) For each HOME-assisted unit, the unit size, household size, <br /> household income, income limits for household, rent payment for tenant <br /> household , utility allowance, and total monthly rent. <br /> REV: 06-28-16 VR <br /> Page 21 of 37 <br /> ATTY/AGR.2016. 169/HHDC Pine-Middlefield Rehabilitation <br />
