A Waiver of Subrogation or Right to Recover endorsement in favor of the State must be attached to certificate. If applicable, Grantee shall provide coverage for all its employees for any injuries or claims under the U.S. Longshoremen's and Harbor Workers' Compensation Act, the Jones Act or under laws, regulations, or statutes applicable to maritime employees. By signing this contract, Grantee acknowledges compliance with these regulations. e. Environmental/Pollution Liability Grantee shall maintain Pollution Liability for limits not less than $1 ,000,000 occurrence covering the Grantee's liability for bodily injury, property damage and environmental damage resulting from pollution and related cleanup costs incurred arising out of the work or services to be performed under this contract. The policy must include: "The State of California, its officers, agents, employees and servants as additional insureds, but only with respect to work performed under the contract." This endorsement must be supplied under form acceptable to the Office of Risk and Insurance Management. Coverage shall be provided for both work performed on site and during transportation as well as proper disposal of hazardous materials. 3. Self-Insurance If the Grantee is self-insured for a portion or all of its insurance, the Grantee shall provide evidence of self-insurance when requested by DBW. Review of financial information including a letter of credit may be required . The Division reserves the right to request financial information. 4. Statement of Insurance Coverage: Grantee certifies and agrees that they have all required insurance coverages as stated in the grant agreement, which will be in effect for the entire term of the agreement. DBW may terminate this Agreement for any reason upon thirty (30) days written notice to Grantee. b. If the Grantee fails to keep the required insurance in effect at all times during the term of this agreement, DBW may, in addition to other remedies it may have, terminate this agreement upon two days written notice. c.
