AGREEMENT BETWEEN <br /> FIRST 5 SAN MATEO COUNTY, REDWOOD CITY 2020 <br /> AND THE CITY OF REDWOOD CITY <br /> (SOCIOS FOR SUCCESS) <br /> EXHIBIT A <br /> SCOPE OF WORK <br /> Pursuant to the Agreement for Services between First 5 San Mateo County, Redwood City 2020 and the City of Redwood <br /> City, effective as of August 1 , 2016 the Grantee shall provide Services as described in this Exhibit Al over the term of <br /> this Agreement. <br /> The Grantee commits to align its program with First 5 San Mateo County's strategic approach to collaborate with existing <br /> services. In doing so, resources are maximized with the goal of implementing comprehensive and integrated services <br /> which support the Commission's adopted Strategic Plan for children and families in San Mateo County. <br /> Attachments Al and A2 to Exhibit A reflect the detailed scope of work for the term of this Agreement. <br /> Under this Agreement, the Grantee will create capacity and leadership among family-serving agencies and parent <br /> leaders by launching Community Mobilization Teams (CMT) in unincorporated Redwood City and North Fair Oaks to <br /> increase resource utilization in the areas of food, health, literacy, and early childhood education, by 0-5 families. <br /> Specifically, the Grantee will: <br /> 1 . Build a semi-formal network of parent connectivity and best practices of family engagement in the <br /> community; <br /> 2. Recruit and train fifty 0-5 service provider organization staff in family engagement best practices; <br /> 3. Train 4 neighborhood-based CMTs comprised of 0-5 families (20 parents/caregivers per team) in Redwood <br /> City and North Fair Oaks; <br /> 4. Establish peer-to-peer accountability, coaching, and sustainability for parent leaders involved in CMTs; <br /> 5. Deploy CMTs in Redwood City and North Fair Oaks to connect 2,016 — 3,536 families of 0-5 children to <br /> under-utilized resources in the areas of food, health, literacy, and early childhood education; <br /> 6. Increase the aggregate usage of under-utilized services and resources through community mobilization, <br /> family-to-family outreach, and family-to-family education; <br /> 7. Communicate the importance of the first 5 years of life and collaborate to improve systems integration, <br /> decrease duplicative services, and create lasting, beneficial systems improvements; <br /> 8. Participate in the design of the evaluation tools and in the implementation of data collection activities as <br /> required by the evaluation tools; and <br /> 9.
