6.1.E. - Page 6 <br /> AGREEMENT FOR SERVICE BETWEEN THE REDWOOD CITY SCHOOL <br /> DISTRICT AND <br /> City of Redwood City "Contractor" <br /> This agreement is made as of this day of by and between the Redwood City School District <br /> ("RCSD") and City of Redwood City("CONTRACTOR") to provide services. <br /> NOW, THEREFORE, THE PARTIES AGREE AS FOLLOWS: <br /> 1. SERVICES: In consideration of the payments hereinafter set forth, <br /> CONTRACTOR shall perform services for RCSD in accordance with the terms, conditions and <br /> specifications set forth herein: <br /> The CONTRACTOR agrees to provide services for the PE+ Program at Hawes, Selby Lane, <br /> Garfield, Henry Ford and Adelante Schools in association with the Sequoia Healthcare District <br /> grant for the 2015-2016 school year. These five schools and RCSD are contributing to the PE+ <br /> program as part of the MOU with the Sequoia Healthcare District. The amounts that the individual <br /> schools are contributing towards the PE+program are described in Exhibit A, attached hereto for <br /> informational purposes only. <br /> A. Scope of Services: Increase physical activity during the school day, along with <br /> positive and consistent health messages. <br /> B. Deliverables: City will conduct programming at the 5 schools listed above during <br /> the 2016-2017 academic year. <br /> • 152days of PE+"full program"implementation that includes PE curriculum instruction <br /> two days per week for each class grades K-5 by up to four total coaches (two coaches <br /> per class) and recess/lunch support five days per week with a minimum of 2 coaches. <br /> The "full program" curriculum instruction ends <br /> • 28 days of "recess/lunch only" provided first two weeks of school, and two weeks <br /> following the end of the "full program". Remaining weeks upon agreement between <br /> each school and PRCS. Implementation by a minimum of 2 coaches. <br /> 2. TERM OF AGREEMENT: The term of this agreement is July 1, 2016 through <br /> June 20, 2017. <br /> 3. COMPENSATION: The CONTRACTOR agrees to perform all the services of this <br /> agreement for an amount not to exceed: One Hundred Four Thousand Six Hundred Fourteen <br /> Dollars ($104,614.00). <br /> 4. PAYMENT: The CONTRACTOR shall submit an invoice detailing the services <br /> performed during the billing period at the end of each month.
