An employee required to serve as a juror <br /> may, upon mutual agreement between the City and the employee, have his/her regular <br /> starting or quitting time changed as a result of being called for jury service. <br /> 9.6 Pregnancy Leave of Absence Without Pay <br /> 9.6.1 Pregnancy leave of absence without pay or benefits shall be granted upon request <br /> to non-disabled probationary and permanent female employees for that period of <br /> time requested up to one(1)year. <br /> 9.6.2 Pregnancy leave shall be granted when the following conditions have been met: <br /> 9.6.2.1 The employee shall notify her immediate supervisor as soon as <br /> possible after pregnancy has definitely been determined. Such <br /> notice shall include the tentative dates on which the leave shall begin <br /> and end. <br /> 9.6.2.2 Within thirty (30) days of the beginning of the pregnancy leave, the <br /> employee shall submit to the Personnel Officer the specific date she <br /> intends to begin the leave, accompanied by her physician's written <br /> statement attesting to the employee's ability to continue performing <br /> the full schedule of duties and responsibilities. She shall continue <br /> her regular active duty until the specific date providing she is able to <br /> perform the full duties and responsibilities of her position and <br /> furnishes additional health statements from her physician upon <br /> reasonable request. In the event the employee is unable to perform <br /> the full duties and responsibilities of her position (as mutually <br /> agreed upon by the Chief of Police, the Personnel Officer, and with <br /> the certification from the employee's attending physician), she shall <br /> be assigned to light duty until the specific leave date, and shall <br /> continue to furnish additional health statements from her physician <br /> upon reasonable request. <br /> 9.6.2.3 Prior to the establishment of a specific date for return to duty, the <br /> employee shall submit to the Personnel Officer a notice of intention <br /> to return to duty, accompanied by her physician's statement <br /> certifying that the employee is medically qualified to assume regular <br /> duties and responsibilities. <br /> 9.6.2.4 The Personnel Officer or his/her designee may designate the specific <br /> beginning and ending dates within the pay period requested by the <br /> employee to meet the needs of the employee and the City. <br /> 9.6.3 The employee on leave shall be returned to an equivalent position within her <br /> classification. <br /> 9.6.4 A pregnancy leave, absent physical disability, is granted without pay for the <br /> duration of the leave. The employee may elect to continue medical and dental <br /> insurance coverage for up to one(1) year during that leave at her own expense,. <br /> 25 <br />
