EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Task 1 — Health and Safety Plan <br /> • Contractor will prepare a site-specific project Health and Safety Plan (HASP) and job <br /> safety analyses for the City's review and approval prior to start of pilot test activities. The <br /> HASP will include site security, traffic control measures and temporary no parking zones. <br /> • Contractor will conduct one Health and Safety Loss Prevention Observation (LPO) per <br /> every five days of site activities during the project. Copies of the completed LPOs are to <br /> be provided to the City within one week of the observation. <br /> • Contractor will use, at a minimum, Level D Personal Protective Equipment (PPE) during <br /> the project. Level C PPE should be available at all times should an upgrade be required. <br /> • Contractor shall conduct daily safety meetings prior to initiating any site activities. <br /> Task 2 _ Preconstruction Kickoff Meeting <br /> • Contractor shall attend the pre-construction kickoff meeting. <br /> Task 3 — Site Security and Traffic Control Measures <br /> • Contractor will maintain public safety and minimize impacts to property: <br /> O 1 . Confine all operations to areas permitted by the City <br /> O 2. Do not encumber site with excessive material or equipment <br /> O 3 . Do not impose any load on any structure that will damage or endanger structure <br /> O 4. Take precautions necessary to prevent annoyance to occupants adjacent to or in <br /> the vicinity of the work <br /> O 5. Contractor is solely responsible for conditions of the jobsite, including safety of <br /> all persons and property during performance of work. This applies continuously <br /> and is not limited to normal working hours. Contractor shall maintain safety and <br /> sanitary provisions so that they conform to applicable Federal, State, County and <br /> local laws, regulations, ordinances, standards and codes. <br /> • Contractor will provide all required traffic control measures and temporary no parking <br /> zones for use during pilot test activities. Contractor will furnish and place property safety <br /> cones and physical barricades for the prevention of accidents. Traffic control devices, <br /> will be, at a minimum, in accordance with applicable state and/or local codes. <br /> • Contractor shall rebuild, repair, restore, and make good to the City's satisfaction all <br /> damages resulting from Contractor's operations. <br /> • Contractor will, upon conclusion of field activities, breakdown and remove all materials <br /> and equipment from the site in a timely manner. Contractor will power-wash the asphalt <br /> surface if needed to remove residuals. If power washing is performed, Contractor will <br /> collect and drum wash water for removal by a disposal company. <br /> • Contractor will remove all general refuse from the site for disposal. <br /> Task 4 — Utility Clearance <br /> • Contractor shall arrange for underground utility locating and clearance prior to site <br /> mobilization, and Contractor shall take any and all steps necessary to avoid damaging <br /> existing utilities. <br /> REV: 10-07-16 JS <br /> Page 9 of 13 <br /> ATTY/AGR.2016.294/Cascade Drilling - Groundwater Remediation Pilot <br />
