Delays in submission of reports will delay <br />payments of invoices to Contractor. Quarterly reporting must include the following: <br /> Client services delivered (Clarity Performance Measurement Report). <br /> Narrative describing highlights and challenges. <br /> Detailed expense invoice. <br /> Brief report of any additional funding sources/revenue the agency has secured as well <br />as any decrease in revenue. <br /> Results for each of the measures listed in the Performance Reporting section below will <br />be submitted quarterly. <br /> Customer satisfaction will be reported on a semi-annual basis in the 2nd and 4th quarter <br />reports. <br /> <br />A. Number of Clients Served – by Households and Individuals <br /> <br />Number of clients/participants served each fiscal year.
