EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Funding for this agreement has been granted from the San Mateo County Health System, <br /> Department of Behavioral Health and Recovery Services to support the Redwood City and <br /> North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br /> Friends for Youth ("FFY") will work in collaboration with the AOD Abuse Prevention Partnership <br /> to build program capacity in increasing assets and resilience of vulnerable populations through <br /> youth development, to increase the number of youth and adults in supportive and caring <br /> relationships, and to provide youth development training and support for adult mentors in the <br /> community. <br /> Specific tasks will include: <br /> • FFY will provide at least 5 prevention-focused , evidence-based trainings to at least 20 <br /> new mentors to work with at-risk youth from Redwood City and North Fair Oaks. <br /> Trainings will highlight the importance of positive familial & other relationships and <br /> encourages pro-social behavior development in mentees. <br /> • FFY Program Coordinators will provide a minimum of 30 contacts with caregivers of at- <br /> risk youth (from Redwood City and North Fair Oaks) involved in the program, providing <br /> caregivers with increased knowledge of evidence-based mentoring practices and <br /> prevention-focused activities with the intent to support caregivers mentoring their youth. <br /> • FFY Program Coordinators will provide regular contact (min. 100 contacts) with Mentors <br /> of at-risk youth from Redwood City and North Fair Oaks to provide prevention-focused, <br /> evidence-based support. <br /> • Work with Redwood City 2020 staff and staff at John W. Gardner Center for Youth and <br /> Their Communities to identify indicators of success and develop/implement evaluation <br /> plan for tracking success and impact of all the above-mentioned projects. FFY staff will <br /> continue to improve internal practices in support of mentors and mentees as they relate <br /> to AOD prevention efforts. <br /> • FFY staff will attend monthly Prevention Partnership Steering Committee meetings. <br /> • Ensure appropriate County reporting and data collection requirements are met including <br /> but not limited to entering and maintaining monthly CaIOMS records. <br /> • Complete other tasks related the Redwood City 2020 AOD Prevention Partnership as <br /> assigned and agreed upon by Friends for Youth and Redwood City 2020. <br /> Consultant will invoice RWC 2020 twice per fiscal year, providing documentation of actions <br /> taken during that proceeding invoice period.
