Describe agency/entity responsible for preparing the Consolidated Plan and those <br />responsible for administration of each grant program and funding source <br />The following are the agencies/entities responsible for preparing the Consolidated Plan and <br />those responsible for administration of each grant program and funding source. <br />Agency Role Name Department/Agency <br />CDBG Administrator REDWOOD CITY Community Development <br />Department <br />HOME Administrator REDWOOD CITY Community Development <br />Department <br />Table 1— Responsible Agencies <br />Narrative <br />The Housing Staff of the Planning & Housing Division of the Community Development Department is <br />the Lead Agency to develop and implement the Consolidated Plan Process as well as carry out the <br />Strategies and Priorities for the period of 2015-2017. The Division Supervisor will have primary <br />responsibility for implementation, monitoring and submittal of required performance and evaluation <br />reports during the life of the Plan. <br />The Division Supervisor's efforts will be coordinated with the Human Services Manager, from the Parks, <br />Recreation and Community Services Department. The Human Services Manager shares responsibility for <br />the homeless continuum of care and implements the City's Anti -Poverty Strategy. The Fair Oaks <br />Community Center is one of the County's core referral agencies for the delivery of coordinated human <br />services and the home of over 20 non-profit organizations who deliver services to the Redwood City <br />community. <br />The Housing Staff will work hand in hand with the Community Development Department and the Parks, <br />Recreation & Community Services Department's Fair Oaks Community Center. The Community Center is <br />the home of the City's Social Services Center where Basic Human needs are provided by 20 different <br />public and non-profit organizations who either receive in-kind housing at the facility or provide <br />programs and services at the Community Center on a regular basis. Additionally, the Community Center <br />is co -located with the County's Human Services Agency, further facilitating access to basic social <br />services. <br />The Fair Oaks Community Center is one of San Mateo County's 7 Core Referral Agencies for the delivery <br />of Human Services and programs and services meeting basic human needs, including referrals to shelter, <br />food, transportation and housing intervention.
