<br />, <br /> ORIGINAL <br /> AGREEMENT <br /> [SAMARITAN HOUSE] <br /> THIS AGREEMENT, made and entered into this J to -\1'\ day of August, 2004, by and <br /> between the CITY OF REDWOOD CITY, a charter city and municipal corporation of the State of <br /> California ("City") and SAMARITAN HOUSE, a California nonprofit organization ("Contractor"); <br /> WIT N E SSE T H: <br /> WHEREAS, Contractor provides free healthcare services to low-income individuals and families <br /> who do not receive medical insurance through their low-wage jobs, cannot afford "out of pocket" <br /> healthcare expenses, and have no other access to healthcare services; and <br /> WHEREAS, Contractor has requested financial assistance in order to conduct the program <br /> for residents of City during the fiscal year commencing July 1, 2004, and ending June 30, 2005; <br /> and <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum <br /> of Twenty Thousand and No/100 Dollars ($20,000); <br /> NOW, THEREFORE, the parties hereto do hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Twenty <br /> Thousand and No/100 Dollars ($20,000) for exclusive use by Contractor during the fiscal year <br /> commencing July 1, 2004, and ending June 30, 2005, solely for the purposes described in <br /> paragraph 2. <br /> 2. USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 <br /> solely for the Program purposes described in Exhibit "A" (Contract Objectives) attached hereto <br /> and by this reference incorporated herein. <br /> 3. CHANGES TO PROGRAM. No changes in the Program described in this agreement <br /> which are funded by the financial assistance provided under paragraph 1 shall be made without <br /> the prior written consent of City. <br /> Atty!
