Coordinated Entry/ <br />Shelter <br />Complete assessments of individuals/households seeking shelter. When <br />shelter is needed, contractor will refer to shelter services following the <br />shelter assessment and referral process as established by HSA, <br />including following the Coordinated Entry System to be established by <br />HSA. <br />Complete referrals for additional homeless services. <br />Housing <br />Refer and provide information on affordable housing options. This <br />includes but is not limited to Department of Housing website, <br />SMCHousingSearch.org, Moving -To -Work application online, HIP <br />Housing, MidPen Housing. <br />For veterans who are homeless, also refer to SSVF and VASH program. <br />Food <br />Maintain a supply of non-perishable food on site that can be distributed to <br />clients with immediate food needs. If space limits do not allow food to be <br />kept onsite, contractor will have a referral list of immediate food access <br />points for clients with an immediate need. <br />Utilize Second Harvest and/or other resources to obtain food. <br />Assist clients with applying for and accessing food via Second Harvest <br />and other community food programs. When possible, include access to <br />fresh produce. <br />Financial <br />Assistance <br />Provide rent assistance, mortgage assistance, security deposits, utilities <br />assistance , car repair, and other emergency financial needs using all <br />available funding sources such as Measure A Homeless Prevention <br />Assistance Program, Season of Sharing, Community Services Block <br />Grant (CSBG), Housing Industry Fund (HIF), and other available funding, <br />following the guidelines and eligibility criteria established for each funding <br />source. <br />Refer clients to utility assistance programs and liaise with the appropriate <br />entities such as Central Coast Energy Services to submit the application <br />and assist clients with completing the application process. <br />Material Goods <br />Assist clients with accessing resources for clothing, diapers, and other <br />emergency material needs. <br />Transportation <br />Provide referrals to transportation programs, including Redi-Wheels. <br />Iemplota Ver,mn du5usz 26.2016 <br />Page 11 <br />ATTY/AGR/2016.321/AGREEMENT BETWEEN SAN MATEO COUNTY AND FAIR OAKS COMMUNITY CENTER <br />REV: 11-18-16 JS Page 11 of 25 <br />
