Exhibit C <br />Performance Reporting <br />November 15, 2016- June 30, 2019 <br />City of Redwood City— Fair Oaks Community Center <br />Quarterly reporting is a requirement of payment. Delays in submission of reports will delay <br />payments of invoices to Contractor. Quarterly reporting must include the following: <br />• Client services delivered (Clarity Performance Measurement Report). <br />• Narrative describing highlights and challenges. <br />• Detailed expense invoice. <br />• Brief report of any additional funding sources/revenue the agency has secured as well <br />as any decrease in revenue. <br />• Results for each of the measures listed in the Performance Reporting section below will <br />be submitted quarterly. <br />• Customer satisfaction will be reported on a semi-annual basis in the 2nd and 4t' quarter <br />reports. <br />A. Number of Clients Served — by Households and Individuals <br />Number of clients/participants served each fiscal year.
