EXHIBIT "A" SCOPE OF SERVICES Funding for this agreement has been granted from the San Mateo County Health System, Department of Behavioral Health and Recovery Services to support the Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. StarVista will work in collaboration with the ADD Abuse Prevention Partnership to build program capacity, and to increase the number of youth involved in positive activities and supportive youth/adult relationships. Specific tasks to be completed include: • StarVista staff will collaborate with the Sequoia Youth Advisory Board (YAB) to update the assessment tool with AOD survey questions; • StarVista will coordinate with school staff to allow YAB members to go into classrooms and administer the Needs Assessment Survey with students. Staff and youth will present the outcomes of this survey to the Prevention Partnership and administrative staff at Sequoia High School; • StarVista staff will be trained to implement the Leadership and Resiliency program and will recruit at least 14 high-risk youth at Sequoia High School to participate in this program; • StarVista will facilitate a Leadership and Resiliency program in the 2016/2017 school year at Sequoia High School, implementing the following 4 program components: o weekly resiliency group meetings, including expressive art approaches, psycho- education on drugs and alcohol, as well as individual skill -building; o community service activities; o peer education; 0 outdoor activities; • Work with staff at John W. Gardner Center for Youth and Their Communities to refine indicators of success and implement evaluation plan for tracking success and impact of all the above-mentioned projects; • StarVista staff will attend monthly Prevention Partnership Steering Committee meetings; • Ensure appropriate County reporting and data collection requirements are met including but not limited to entering and maintaining monthly CalOMS records; • Complete other tasks related the Redwood City 2020 AOD Prevention Partnership as assigned and agreed upon by Peninsula Conflict Resolution Center and Redwood City 2020. Consultant will invoice Redwood City 2020 twice per fiscal year, providing documentation of actions. Payment will be two equal installments amounting to the full and total compensation unless otherwise determined pursuant to actions invoiced.
