EXHIBIT "A" <br />SCOPE OF SERVICES <br />Funding for this agreement has been granted from the San Mateo County Health System, <br />Department of Behavioral Health and Recovery Services to support the Redwood City and <br />North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br />StarVista will work in collaboration with the ADD Abuse Prevention Partnership to build program <br />capacity, and to increase the number of youth involved in positive activities and supportive <br />youth/adult relationships. <br />Specific tasks to be completed include: <br />• StarVista staff will collaborate with the Sequoia Youth Advisory Board (YAB) to update <br />the assessment tool with AOD survey questions; <br />• StarVista will coordinate with school staff to allow YAB members to go into classrooms <br />and administer the Needs Assessment Survey with students. Staff and youth will present <br />the outcomes of this survey to the Prevention Partnership and administrative staff at <br />Sequoia High School; <br />• StarVista staff will be trained to implement the Leadership and Resiliency program and <br />will recruit at least 14 high-risk youth at Sequoia High School to participate in this <br />program; <br />• StarVista will facilitate a Leadership and Resiliency program in the 2016/2017 school <br />year at Sequoia High School, implementing the following 4 program components: <br />o weekly resiliency group meetings, including expressive art approaches, psycho- <br />education on drugs and alcohol, as well as individual skill -building; <br />o community service activities; <br />o peer education; <br />0 outdoor activities; <br />• Work with staff at John W. Gardner Center for Youth and Their Communities to refine <br />indicators of success and implement evaluation plan for tracking success and impact of <br />all the above-mentioned projects; <br />• StarVista staff will attend monthly Prevention Partnership Steering Committee meetings; <br />• Ensure appropriate County reporting and data collection requirements are met including <br />but not limited to entering and maintaining monthly CalOMS records; <br />• Complete other tasks related the Redwood City 2020 AOD Prevention Partnership as <br />assigned and agreed upon by Peninsula Conflict Resolution Center and Redwood City <br />2020. <br />Consultant will invoice Redwood City 2020 twice per fiscal year, providing documentation of <br />actions. Payment will be two equal installments amounting to the full and total compensation <br />unless otherwise determined pursuant to actions invoiced. <br />The terms of this agreement will be effective from July 1, 2016 until June 30, 2017. <br />REV: 11-10-16 RL <br />Page 10 of 10 <br />ATTY/AGR.2016.317/AOD PP 2016-2017 StarVista - RC2020 <br />
