<br /> DES:dft 12/17/90 (028/8) <br /> DES:dft 6/11/93R (016/11) <br /> 6/24/93R <br /> 7/22/93R <br />(b) A list of any environmental control permits held <br /> by the applicant for the facility or Premises; <br />(c) A brief description of the nature, average rate of <br /> production and standard industrial classification <br /> of the operation(s) carried out by the applicant; <br />( d) Flow measurement showing average daily and maximum <br /> daily flow from each process stream to which the <br /> application pertains; <br />(e) Wastewater Constituents and Characteristics of the <br /> Wastewater proposed to be discharged into the <br /> Sewerage Facilities, including, but not limited <br /> to, those categories thereof described in Sections <br /> 27.11, 27.12, and 27.13, the presence and amount <br /> of which shall be determined by a laboratory <br /> competent to test and describe such Constituents <br /> and Characteristics, and approved by Authority's <br /> Manager; <br />(f) The time and duration of the proposed Wastewater <br /> discharge; <br />(g) The average and thirty minute peak Wastewater flow <br /> rates proposed to be discharged, including daily, <br /> monthly, and seasonal variations, if any; <br />(h) Site plans, floor plans, mechanical and plumbing <br /> plans, in detail necessary or appropriate to show <br /> and to describe all Building Sewers and <br /> appurtenances by size, location and elevation; <br />(i) A description of the activities, facilities, and <br /> plant processes conducted, or proposed to be <br /> conducted on the Premises, including, but not <br /> necessarily limited to, all materials <br /> manufactured, fabricated, or processed, and the <br /> types of materials which are or could be <br /> discharged into the Sewerage Facilities; <br />(j) Identification of Pretreatment standards <br /> applicable to each process; <br />(k) A statement, reviewed by an authorized <br /> representative of the applicant and certified by a <br /> qualified professional, stating whether <br /> Categorical Standards are being or will be met on <br /> a consistent basis and, if not, whether <br /> additional operation and maintenance and/or <br /> additional Pretreatment is required for the <br /> applicant to meet such standards and requirements; <br />(1) Requirements, if any, for additional Pretreatment <br /> 26 <br /> ....
