EXHIBIT "A" <br />SCOPE OF SERVICES <br />1. Scope of Work <br />Consultant shall provide the following services. <br />A. LEADERSHIP COACHING SERVICES: <br />• Consultant shall facilitate initial session with Coaching Client and his Manager to <br />outline coaching process and begin determining development focus areas and <br />desired results. <br />• Consultant shall co -create and implement a Leadership Development Plan <br />(LDP), including vision statement, and coach the Coaching Client in obtaining <br />approval from his Manager. <br />• Consultant shall provide instructions and link to the online Emotional Quotient <br />Inventory (EQ -1 2.0) and debrief EQ -1 Leadership Report. <br />• Consultant shall conduct 2 check -ins with Coaching Client and his Manager to <br />debrief progress with LDP, modify course if needed, and/or formulate transition <br />plan from coaching for ongoing support and accountability. <br />• Consultant shall provide 10 hours of individual 1:1 coaching over 6-12 <br />consecutive month period in-person or by phone beginning in March 2017. <br />• Consultant shall provide additional phone and/or email coaching between <br />sessions as needed/requested and within limits reasonable for the coaching <br />process. <br />• Consultant shall prepare for coaching sessions and provide materials as <br />needed/requested. <br />B. 360 FEEDBACK SERVICES -Leadership EQ 360 <br />• The Leadership EQ is an online assessment of emotional intelligence that <br />provides a 360 -degree view, combining the Coaching Client's self-evaluation with <br />selected contributors — manager, peers, direct reports, friends/family/other. <br />• Consultant shall co -select 6-12 contributors, identify 3-5 pertinent, open-ended <br />questions for response, administer the online assessment, and deliver and <br />debrief the confidential Leadership 360 Feedback Report to the Coaching Client <br />only. <br />2. Payment Terms <br />Consultant shall provide the Leadership Coaching Services for a total cost of $7,500. <br />Leadership Coaching Services shall be invoiced at $1,875 monthly for 4 consecutive <br />months.
