CGM:mf 01/26/83 ;M:mf 02/14/83R CGM:djg-c 10/18/83R CGM:rb 11/Ol/83R CGM:djg-c 12/23/83R DES:rb 1/23/84R CATV underground system by the Grantee for a proposed subdivision, so that the subdivider may incorporate such information into the plans and specifications for the subdivision to be constructed and may provide the Grantee the opportunity of placing conduit into the streets of the proposed subdivision in connection with the placing of conduits for other utilities. "Section 11.87. (a) Applications for a franchise hereunder shall be filed with the City Clerk in a form approved by the City and shall contain the following information: " (1) The name and address of applicant. " (2) A general description of applicant's proposed CATV operation, including a map of areas to be served. " (3) A statement or schedule in a form approved by the City of proposed rates and charges to subscribers for installation and services, and a copy of a proposed service agreement between the Grantee and its subscribers shall accompany the application. For unusual circumstances, such as the requirement for underground cable, or where there is more than one hundred fifty (150) feet between the cable lines to the service connection for a subscriber, an additional installation charge above that normally charged for installation as specified in the applicant's proposal may be charged, with the requirement that necessary easements shall be supplied by affected subscribers. For remote, relatively inaccessible subscribers within the City, service shall be made available on the basis of cost of materials, labor, and easements, if required. " (4) A statement of the corporate organization of the applicant, including the names and addresses of its officers, directors and associates, and also including the names of subsidiary companies with a listing of of other areas being served by CATV or similar systems.
