IW1 <br />CITY OF REDWOOD CITY <br />Health Connected <br />FISCAL YEAR 2016-2017 <br />Agency Information <br />Agency Name: Health Connected <br />Address: 480 James Ave <br />City, State, Zip: Redwood City, CA 94062-1041 <br />Activity/Program Title: Redwood City Reproductive Health Education <br />Project <br />Quantitative Goals <br />Description of Services to be Provided: <br />During the 2016-2017 school year, Health Connected expects to provide in - <br />school comprehensive sexual health education courses to approximately 2,350 <br />5th -12th grade students in Redwood City. <br />Through this project, Health Connected's highly trained health educators will <br />provide to local students five developmentally -appropriate courses in sexual <br />health education - Puberty Talk, Teen Talk Middle School, Teen Talk High <br />School, Teen Talk High School Refresher, and, currently in development, Teen <br />Talk Special Education. <br />The goals of this project are to improve young people's lives by increasing their: <br />- Knowledge about pregnancy and sexually transmitted infection prevention <br />- Skills for avoiding unwanted sexual activity <br />- Ability to access health services <br />- Engagement with parents/guardians in providing sexual health education <br />Primary Quantitative Objectives: <br />The Redwood City Reproductive Health Education Project will serve 2,350 <br />Redwood City persons (youth). <br />NOTE: The grant performance period begins July 1, 2016 and concludes June 30, <br />2017 <br />REV: 03-24-17 JS <br />Page 9 of 9 <br />ATTY/AGR.2017.061/HSFA - Health Connected <br />
