EXHIBIT A <br />1. Scone of Work <br />Contractor will provide diving services for five potable water tanks in Redwood City, California: <br />• Sequoia Tank #1; <br />• Sequoia Tank #2; <br />• Cambridge Tank#1; <br />• Cambridge Tank #2; and <br />• Easter Bowl Tank. <br />For each tank, Contractor will: <br />1. Coordinate with City staff to select appropriate day(s) for tank diving and cleaning. <br />2. Mobilize to and from the project site. <br />3. Set-up and breakdown equipment. <br />4. Prepare for dive. <br />5. Remove up to three (3) inches of sediment from the tank floor of each tank. <br />6. Visually inspect the interior and exterior of each tank. <br />7. Provide written reports with photos for each tank, summarizing the results of the visual <br />inspections and sediment removal activities. <br />8. Provide a DVD of each dive to City staff. <br />9. Use de -chlorination tablets prior to discharging any water from the tanks. <br />2. Pavment Terms <br />The total not to exceed contract amount is $20,000. The City agrees to pay a flat rate of $16,735 <br />for all work outlined herein on the five potable water tanks identified below. Additional charges <br />for sediment bags and any overtime, if necessary and approved by the City, will be charged at <br />the rates indicated below. <br />Tanks Tank Description <br />1 4MG, Sequoia Tank #1, on -grade, concrete, 165.5 ft. diameter, 26 ft. high <br />1 4MG, Sequoia Tank #2, on -grade, concrete, 165.5 ft. diameter, 26 ft. hi£�h 1 <br />1 510KG, Cambridge Tank #1, on -grade, steel welded, 59 ft. diameter, 25 $. high <br />1 132KG, Cambridge Tank #2, on -grade, steel welded, 30 ft. diameter, 25 ft. high <br />1 1.2MG, Easter Bowl Tank, on -grade, steel welded, 84 ft. diameter, 32 ft. high j <br />Total $16,735.00 <br />Sediment bags will be invoiced at $92.00 for small bags and $124.00 for large bags. Costs for <br />sediment bags will be shown as a separate line item on the final invoice. <br />Overtime services will be performed at the rate of $440.00 per overtime hour if overtime is two <br />hours or less.
