<br />T[C Administrators Self-Funded Quote <br />mE <br />LIPMAN <br />COMPANY, INe. <br /> GENERAL ADMINISTRATIVE DUTIES: <br /> TLC Administrators will... <br /> · Provide the staff, office equipment and space necessary to accomplish the <br /> administrative functions required. <br /> · Staff the administration office to answer all telephone calls, mail, personal inquiries <br /> or communications from employers, employees or others with respect to the <br /> requirements and claims paying procedures of the plan, including (but not limited to) <br /> the details of the manner in which claims should be submitted. <br /> · Routinely make presentations and provide information on benefit coverage at <br /> designated times to participants, employer groups and companies at the business <br /> site within a 60-mile radius. <br /> · Maintain all correspondence, claim files, insurance contracts, plan documents, trust <br /> agreements and any other documentation necessary for the operation of the plan. <br /> · Update, redesign and prepare all claims administration forms (such as, claim forms <br /> and worksheets) as needed. The cost of printing such forms is included in the <br /> administration fees. <br /> · Pursue and maintain positive relations with dentists through direct solicitation of <br /> benefit program acceptance and prompt claim payment. <br /> · Maintain a direct relationship with employers and participating members to promote <br /> the plan. <br /> · Assist the consultants in preparing the annual claims experience report of the plan, <br /> including supplying all claims information in the format required to determine proper <br /> reserves and benefit funding levels. <br /> · Prepare all reports required by law, including 1099 forms for all providers and <br /> ERISA compliance documentation. Assistance of the accountants, legal counsel <br /> and actuary will be solicited as appropriate. <br /> · Arrange for the storage of plan records at the cost of the client. <br /> 3 <br /> .,., ·r <br />
