Prepayment fees payable by <br />Employer to OHS are calculated on a capitated basis based on the <br />number of Members enrolled as of the first day of each month <br />during the term of this Group Agreement as follows: <br /> $2.21 per Subscriber per month. " <br /> 2. City and OHS agree that Section 3 of Addendum Three, <br />Benefits and Services, is hereby delet.ed in. its entirety and the <br />following is inserted in its place: <br /> " 3 . Benefits and services include: <br /> a) Six (6 ) counseling sessions per family unit <br /> b) Management referral/consultation up to a maximum <br /> of six (6 ) seSSlons <br /> c) Critical Incident Debriefing provided as needed <br /> for fire department personnel involved ln a <br /> traumatic incident at no additional charge. <br /> Critical Incident Debriefing for all other <br /> personnel provièed as needed at the rate of <br /> $150.00 per hour plus travel <br /> d) Pr::Jgram materials to include brochures, posters, <br /> payroll stuffers and monthly newsletters <br /> e) One ( 1 ) employee orientation <br /> f) One (1 ) supervisory training session <br /> g) Quarterly statistical reports". <br /> 3 .
