VISION SERVICE ~ COSTS UNDER THE PLAN PREPAYMENT FEES Payments by contract holder to VSP for services and materials provided hereunder during the Initial Term of the contract shall be as follows: $12.95 per month for each eligible employee (includes coverage for eligible dependents) The entire cost of the program is paid to VSP by the employer (contract holder) . Payments are due on the first day of each consecutive month. After the Initial Term of the contract, VSP may, upon sixty (60) days' prior written notice, postage prepaid to Group, change the above cost, provided that VSP shall not raise the cost more often than once in any twelve (12) month period. Notwithstanding the above, VSP reserves the right to increase the payments provided for in this agreement by the amount of any tax or assessment not now in effect, subsequently levied upon VSP by any state or other taxing authority which is attributable to the payments VSP receives from Group. In the event of such an inc~ease, VSP shall provide Group witb thirty (30) days' advance notice. DEDUCTIBLE The benefits described herein are available to each covered person from any participating VSP Panel Doctor at no cost to the covered person, provided covered person follows the proper procedures by obtaining a VSP benefit form and presenting it to the doctor in advance.
