<br /> AMENDMENT TO AGREEMENT <br />WITH FAIR OAKS INFORMATION AND REFERRAL CENTER <br /> FOR HUMAN SERVICES PROGRAM <br />~'·~"IIt·.""'."'" <br />This AMENDMENT TO AGREEMENT is hereby made and entered intoJtl!1 ðf- <br />J~ by and between County of San Mateo and the Fair Oaks <br />Information and Referral Center: <br />WHEREAS, the parties have previously entered into an Agreement dated <br />December 7, 1993; and <br />WHEREAS, the parties desire to amend the Agreement as provided herein; NOW <br />THEREFORE, the parties agree as follows; <br />1) C.1 Maximum Amount of the agreement is hereby amended to read: "The <br /> maximum amount of this contract shall not exceed $37,248." <br />2) C.S Effective Period and Right of Termination is hereby amended to read: <br /> "Unless terminated as set forth in this paragraph, the term of this agreement <br /> is from July 1, 1993 through June 30, 1994." <br />IN WITNESS WHEREOF, the parties hereto have caused this Amendment to the <br />Agreement to be executed on the day and year of this Agreement first above written. <br />r - <br />
