<br /> SAN MATEO COUNTY HUMAN SERVICES AGENCY <br /> EXHIBIT A <br /> CITY OF REDWOOD CITY <br /> FAIR OAKS COMMUNITY CENTER <br /> INFORMATION AND REFERRAL PROGRAM <br /> CONTRACTOR <br />In consideration of payments herein provided for, Conttactor shall, under the general direction <br />of the Director of the Human Services Agency, or her authorized representatives, provide for <br />the following services to benefit the citizens of San Mateo County as described below: <br />THE CITY OF REDWOOD CITY, FAIR OAKS COMMUNITY CENTER INFORMA nON <br />& REFERRAL PROGRAM IS A MUNICIPAL CORPORATION OF THE STATE OF <br />CALIFORNIA OPERATING A HUMAN SERVICES ASSISTANCE PROGRAM IN SAN <br />MA TEO COUNTY FOR LOW INCOME PEOPLE. <br />CITY OF REDWOOD CITY, FAIR OAKS COMMUNITY CENTER INFORMATION & <br />REFERRAL PROGRAM WILL HEREIN BE REFERRED TO AS "CONTRACTOR". <br />CITY OF REDWOOD CITY, FAIR OAKS COMMUNITY CENTER INFORMATION & <br />REFERRAL PROGRAM, 2600 MIDDLEFIELD ROAD, REDWOOD CITY, CALIFORNIA <br />94063, OPERATING MONDAY THROUGH FRIDAY, 8:00 A.M. to 12:00 NOON AND <br />1:00 P.M. TO 5:00 P.M. <br />I. CLIENT SERVICES: <br />The Conttactor will provide the following services, supporting activities, and related information <br />for a two year period from July 1, 1994 through June 30, 1996. <br />Services Provided: Contractor serves as the primary access point for information on resources <br />available to low income persons in Spanish and English in the Fair Oaks community; provides <br />needs assessment, crisis intervention, short-term emergency material assistance and ttanslation <br />services. Serves as a referral site for the County Shelter Network program and intake site for <br />Season of Sharing Assistance for the service area, These services will be provided in an <br />environment that respects the dignity of the client and endeavors to foster independence and self- <br />sufficiency. <br />The following number of clients to be served are estimates only. Conttactors may provide <br />services which vary no more than 10% under the estimated number to be served. <br /> 1. 8028 contacts with residents of the County by phone or in the office for purposes of <br /> giving information (4088), providing needs assessment (2188), and followup (2188). <br /> 2.
