<br />PUBLIC WORKS SERVICES <br />2017 Redwood Shores Lagoon Aquatic Plant <br />Management and Environmental Engineering Services RFP 7 <br /> Safety Training: describe your training program, training resources utilized, <br />initial training plan and continuing training plan. <br /> Shop, Storage, Equipment, and Vehicle Inventory. <br /> <br />5.5. Project Staffing <br />Discuss how the Proposer will staff this project. Key project team members shall be <br />identified by name, title, and specific responsibility for the project. Resumes for all of <br />the Proposer’s personnel shall be included. <br /> <br />Provide documentation or copies of applicable certifications and / or licenses from the <br />California Department of Pesticide Regulations required to apply chemical treatments, <br />particularly aquatic pest control certification or license. Firms that do not possess the <br />necessary licenses from the California Department of Pesticide Regulation, at the time of <br />proposal submittal, shall be deemed non-responsive and shall be disqualified. <br /> <br />5.6. Proposal Cost <br />Proposer will provide a projected number of hours required per month to complete the <br />scope of work, and the estimated dollar amount associated with these hours. A sample <br />schedule is provided in Attachment A (Proposed Work Plan). The proposer will not <br />exceed the budgeted monthly dollar amount without written approval from an approved <br />agent of The City. The City will not be required to pay any invoice exceeding the budgeted <br />monthly dollar amount if approval from The City is not obtained by the proposer. <br /> <br />The Proposer will be responsible for obtaining any materials required to perform work. <br />In this section the proposer will provide the standard markup rate (if any) for all materials <br />procured on behalf of the City. <br /> <br />6. Contract Type, Term and Method of Payment <br />It is anticipated that the contract resulting from this RFP, if awarded, will be a pay as you go contract. <br />The term of the agreement is two (2) years; and, the City may, at its sole discretion, extend the term <br />of the agreement for a period of four (4) years in increments of one year each. After work has been <br />approved and performed, services will be invoice to Redwood City Public Works Department on a <br />timely basis no later than 30 days after completion of work. The City will not be bound to pay any <br />invoice earlier than 30 days after being receved. <br /> <br />7. Indemnification <br />Consultant will defend, indemnify and hold harmless City and its officers, agents, employees and <br />volunteers from and against all claims, damages, losses and expenses including attorney fees arising <br />out of the performance of the Services, caused in whole or in part by the willful misconduct or any <br />negligent act or omission of the Consultant, any subcontractor, anyone directly or indirectly <br />employed by any of them or anyone for whose acts any of them may be liable, except where caused <br />by the sole negligence or willful misconduct of City. <br /> <br />6.1.B. - Page 19
