<br /> SIDELETTER OF AGREEMENT <br />Sascha Eisner <br />Worksite Organizer <br />Service Employees International Union, Local 715 <br />891 Marshall St. <br />Redwood City, CA 94063 <br />SUBJECT: SIDELETTER OF AGREEMENT <br />Dear Sascha: <br />During the recently concluded negotiations for a successor Memorandum of <br />Understanding, the parties agreed to the following: <br />Eauitv Adiustment: By March 1, 1998, the Union and the City agree to have met and <br />conferred regarding job description, internal benchmark and external market adjustment <br />for the following classification represented by SEIU: <br /> Community Service Officer <br />Salary adjustment shall be effective retroactive to February 1, 1998. <br />TLC Dental: <br />1) TLC will continue to be the plan administrator (with a $2,000 annual cap for basic <br /> coverage and a $1,000 lifetime cap for orthodonture) and will provide the same <br /> level of benefit and coverage as currently provided by Delta Dental for each plan <br /> year throughout the term of the MOU. <br />2) On an ongoing basis, the Human Resources Department agrees to review any <br /> employee claims of discrepancies between the level of benefit available through <br /> Delta Dental and the actual level of coverage provided by TLC. Where such <br /> discrepancies are found to be inconsistent with the commitment in #1 above: <br /> (a) the City will arrange for TLC to make any adjustments necessary to match <br /> the Delta Dental level of coverage then available, and <br /> (b) the employee is entitled to resubmit claims to TLC for reimbursement. <br />3) It will be the responsibility of the employee to provide a copy of the explanation <br /> of benefits from Delta Dental showing the amount allowed for the same type of <br /> service in order to receive the higher allowable expense. <br /> 51 <br />
