<br /> EXHmIT B <br /> CONTRACT OBJECTIVES <br /> PENINSULA VOLUNTEERS, INC. <br /> ROSENER HOUSE <br /> FISCAL YEAR 1995196 <br />This program includes meaningful activities for the various interests and functioning levels. Health <br />monitoring is provided by our registered nurse and individual counseling is provided by our social <br />worker. The nutrition program consists of a hot lunch and morning and afternoon snacks. <br />Transportation is available through Sam Trans' Redi-Wheels program. A plan care is written for each <br />participant and referrals are made to other community resources as needed. While an increasing <br />number of clients served are Alzheimer's Disease victims, other problems include other forms of <br />dementia, stroke, Parkinson's Disease, hypertension, cancer, psychiatric problems, visual impairments, <br />and the entire range of diseases affecting the elderly. <br />Since Redwood City does not have its own adult day support center it is served by Rosener House and <br />Catholic Charities I program in San Carlos. <br /> # of Personsl <br />Goals Households Complption Date <br />See helow 43 6/30/96 <br />1. To provide an adult day support program five days a week from 8 a.m. to 5:30 p.m. to <br />physically/cognitively/impaired older adults living in Redwood City. <br />2. To provide annually services to approximately 43 Redwood City residents for a total of <br />approximately 3,000 attendance days. <br />3. To accept Redwood City residents into the program regardless of ability to pay the fee for service. <br />Approximately 58 % of Redwood City residents will need reduced fees. <br />4. To serve as a congregate meal site serving annually approximately 3,000 hot lunches and morning <br />and afternoon snacks. <br />5. To arrange for an average of 350 one-way rides per month for Redwood City residents to and from <br />Rosener House on Sam Trans Redi-Wheels. <br />6. To provide two monthly support groups for family caregivers. <br />7. Rosener House social worker to provide case management and referrals to other community <br />resources as needed. <br />8. To continue involvement with other service providers through active membership in the Adult Day <br />Services Network of San Mateo County, the Commission on Aging I s Long Term Care Committee, <br />the New Beginnings Coalition of San Mateo, and the California Association of Adult Day Services. <br />
