This insurance shall include, but not be limited to, premises and operations; <br />contractual liability covering the indemnity provisions contained in this Agreement; personal <br />injury; explosion, collapse, and underground coverage, products and completed operations, and <br />broad form property damage. <br /> <br /> Pdor to commencing work or entering onto the Property, QWEST shall file a Certificate(s) <br />of Insurance or declaration page(s), as requested by the Director, with the City Clerk evidencing <br />coverage. Said Certificate(s) and declaration pages shall stipulate: <br /> <br /> · The insurance company(les) issuing such policy(les) shall give written notice to the <br /> City Manager of any material alteration, or reduction in aggregate limits, if such limits <br /> apply, and provide at least thirty (30) days notice of cancellation. <br /> <br /> · That the policy(les) is(are) Primary Insurance and the insurance company(les) <br /> providing such policy(les) shall be liable thereunder for the full amount of any loss or <br /> claim which QWEST is liable for under this Section, up to and including the total limit <br /> of liability, without right of contribution from any other insurance effected or which may <br /> be effected by the City. <br /> <br /> · The policy(les) shall also stipulate: Inclusion of the City, its Council, commission, <br /> boards, committees, officers, employees, and agents as an additional insured shall <br /> not in any way affect its rights either as respects any claim, demand, suit, or judgment <br /> made, brought or recovered against QWEST.
