EXHIBIT A <br /> 1 . Scope of Work <br /> Contractor will provide laboratory analyses of samples collected by the Public Works Services <br /> Department (PWS) in accordance with standards and procedures set forth by the United States <br /> Environmental Protection Agency (USEPA) and the State of California, including but not limited <br /> to the California Code of Regulations and State Water Resources Control Board guidelines. <br /> Contractor will also provide the following: <br /> A. Courier Service <br /> Contractor will provide delivery and pickup of prepared sample bottles/containers and chain of <br /> custody forms to and from PWS (1400 Broadway Street, Redwood City, CA 94063). Depending <br /> on the type of sample, sample delivery and pick-up may be regularly scheduled or requested on <br /> an as-needed basis. Contractor will provide all necessary transport containers and ice to preserve <br /> samples. <br /> Contractor will deliver and/or pick-up samples Mondays through Fridays, between 7:30 AM and <br /> 3 :00 PM. Contractor must pick-up samples within twenty-four-hour notice, and deliver samples <br /> no later than seventy-two hours after receiving a request. <br /> B. Notification of Sample Results <br /> Contractor will provide timely electronic reports of sample results to PWS, especially for <br /> drinking water analyses reports. Reports will be provided electronically to PWS within the <br /> designated standard, or rush, turnaround times specified below in Payment Terms. Contractor <br /> will apply rush turnaround times upon request by the City. <br /> Contractor will upload drinking water and recycled water sample results online to the City's <br /> WaterTrax account. <br /> In the event that sample results exceed regulatory guidelines, Contractor will follow the State <br /> Water Resources Control Board's Division of Drinking Water (SWRCB DDW) procedures for <br /> notification. <br /> C. Electronic Data Transfer <br /> Contractor will submit regulatory results electronically to the SWRCB DDW through Electronic <br /> Data Transfer (EDT). <br /> D. Chain of Custody Procedures <br /> All samples to be analyzed by the Contractor will be accompanied by a chain of custody (COC) <br /> form. In the event COC information is in error or there is a question or clarification regarding <br /> information on the COC, Contractor will contact PWS immediately and prior to proceeding with <br /> the analysis.
