Collaborative partners support each school in hosting a school RC 2020 Event / K Orientation progress in meeting their stated <br />readiness parent / child event to connect families to school goals from the beginning to the <br /> Logs end of treatment <br />8. Collaborative partners continue to identify opportunities to involve <br />the Early Care and Education Committee in the work of RC 2020 and RC 2020 & Meeting minutes <br />the RCFCs. RCFCs 60% of families receiving <br /> referrals for health, <br /> developmental, or social issues <br /> will follow up those referrals. <br /> Systemic barriers to follow- <br /> through on referrals will be <br /> documented. <br /> Partners annually identify <br /> collaborative strengths, <br /> opportunities for growth, and <br /> plans for continuous quality <br /> improvement, using the Self <br /> Assessment Protocol for <br /> Community Collaboration. <br />
